Welcome to WNY Wellness! To help make your first visit as smooth as possible, we offer the option to complete your new patient paperwork ahead of time.

Please download and print the forms below, fill them out at your convenience, and bring them with you to your scheduled appointment at either our Lockport or Williamsville office.

If you prefer to complete your forms in the office, please arrive 15 minutes prior to your appointment time.
